Requesting Records
You have the right to request and receive a copy of your health care records, subject to applicable state and federal laws.
Your rights include:

If you receive substance use disorder services, your records are protected under 42 CFR Part 2 and generally cannot be disclosed without your written consent, except as permitted by law and you may refuse consent or request restrictions for sharing SUD records.
