Requesting Records

You have the right to request and receive a copy of your health care records, subject to applicable state and federal laws. 

 

Your rights include:

• ·       Access and obtain copies of your health information

• ·       Request amendments or corrections

• ·       Request restrictions on disclosures

• ·       Request confidential communications

• ·       File complaints with our office or the appropriate regulatory authority
